In your initial email, please provide:

• Date of birth
• Contact telephone number
• Name and contact details of your new GP/medical practice
For privacy and confidentiality, please do not send sensitive medical information or copies of identification documents in your initial email. We may contact you for identity verification and written authority before releasing or transferring your medical records.
Medical records will be handled and transferred in accordance with applicable privacy and health-record requirements.
